Every individual is unique, and so are their care needs. We collaborate with each client and their families to create an Individual Service Plan (ISP) that caters to each person’s needs and is informed by their personal wants and goals. Our caregivers are dedicated to understanding preferences and providing services that cater to each person’s requirements, scheduling needs, and personalities.
Our services may be described as in-home care or homemaker/personal care services. Depending on the needs of each person, the Individual Service Plan might include:
Personal Care Assistance
Help as needed with everyday self-care tasks such as bathing, dressing, grooming, or maintaining personal hygiene.
Meal Planning & Preparation
Meal planning collaboration and/or assistance in food preparation.
Household Support
Light housekeeping, help grocery shopping, laundry assistance, etc.
Non-Emergency Transportation
Driving to and from medical appointments, leisure activities, and other outings.
Medication Management
Assistance in the correct and timely administration of medication, potentially including medication reminders, help administrating medications, or assistance reading and understanding medication requirements.
Community Engagement & Recreation
Encouraging personal relationships, connecting to local community programs according to the person’s interest, needs, and goals, and support for the exploration of and continued participation in personal hobbies and recreational activities.
